The pending EVOKE and EVOKE Plus trials of daily oral semaglutide participants aged 5585 years with mild cognitive impairment or mild dementia due to Alzheimers disease with confirmed amyloid abnormalities by PET scan or cerebrospinal fluid biomarkers The only difference in the 2 trials is with respect to entry criteria with EVOKE Plus that allows for small vessel pathology
The relatively small sample size and pilot nature of the study may have limited the ability to detect subtle differences between groups and may increase the risk of type I statistical error and effect-size inflation
Risk factors that warrant additional consideration include: Being in the dose-escalation phase of semaglutide therapy Experiencing significant gastrointestinal symptoms (nausea, vomiting, fullness) Taking higher doses of the medication Having known gastroparesis or diabetic autonomic neuropathy For routine dental procedures such as cleanings, fillings, or simple extractions performed with local anesthesia only, semaglutide does not pose additional risks, and patients can continue their regular medication schedule
